This is the list of all the surgeries I've had.

June 24, 1964 - Lip closure at 9 days of age (St. Luke's Hospital)
March 24, 1966 - Palate closure at 21 mos. of age (Research Hospital)
1967 or 1968 - First set of ear tubes (St. Mary's Hospital)
1969 - Construction of first nasal passage at 4 yrs. of age (Research Hospital)
Next day - Sent back to OR because nasal packing came out (Research Hospital)
Sept. 11, 1969 - Surgery to prepare for nasal reconstruction at 5 yrs. of age (Research Hospital)
Sept. 18, 1969 - Surgery to build first nose - Built from flap of skin from left arm at 5 yrs. of age Spent two weeks in a head-to-waist body cast (Research Hospital)
Oct. 2, 1969 - Release of flap at 5 yrs. of age (Research Hospital)
July 30, 1970 - Dilation of lacrimal duct (tear) at 6 yrs. of age (Research Hospital)
August 27, 1970 - Dilation of lacrimal duct (tear) at 6 yrs. of age (Research Hospital)
Nov. 19, 1970 - Bone graft to the nose - reconstruction of columella at 6 yrs. of age (Research Hospital)
April 15, 1971 - Exploration of lacrimal apparatus at 6 yrs. of age (Research Hospital)
August 2, 1971 - Extirpation of lacrimal duct at 7 yrs. of age (Research Hospital)
Summer of 1972 or 1973 - Tighten muscle in left eye at either 8 or 9 yrs. old (Research Hospital)
School year 1973-'74 - Ear tubes at 9 yrs. old (Children's Mercy Hospital)
School year 1974-'75 - Ear tubes at 10 yrs old (Baptist Memorial Hosptial)
August 1976 - Ear tubes at 12 yrs. old (Baptist Memorial Hosptial)
August 1977 - Oral surgery at 13 yrs. old (Baptist Memorial Hosptial)
Oct. 30, 1978 - First stage of nasal reconstruction, midface advancement, and abbe flap at 14 yrs. old (Children's Mercy Hospital)
Nov. 15, 1978 - Release of forehead graft (used to build nose) and abbe flap at 14 yrs. old (Children's Mercy Hospital)
Dec. 15, 1978 - Removal of arch bars (jaws were wired since Oct. surgery) and last set of ear tubes at 14 yrs. old (Children's Mercy Hospital)
Jan. 25, 1979 - Dilation of nostrils (nasal tubes inserted) at 14 yrs. old (Research Medical Center)
Feb. 27, 1980 - Bone grafting (nose and orbital floors, maxilla, and hard palate), lip revision at 15 yrs. old (Children's Mercy Hospital)
March 6, 1981 - Bone grafting to maxill and left orbit, cartilage taken from right ear for nose, genioplasty, lip revision, and pharyngeal flap at 16 yrs. old (Children's Mercy Hospital)
Oct. 29, 1981 - Revisions at 17 yrs. old (Children's Mercy Hospital)
July 9, 1982 - Eye surgery - Tighten muscle in right eye at 18 yrs. old (Children's Mercy Hospital)
Oct. 29, 1982 - More bone grafting to left orbit and maxilla, nasal revision, skin graft for superior labial sulcus with fitting of dental prothesis (space to make my partial fit tight in front), lip revision at 18 yrs. old (Children's Mercy Hospital)
June 20, 1984 - Gum surgery (done in dentist office) at 20 yrs. old (peridontist's office)
Oct. 16, 1987 - More bone grafting to left orbit (bone taken from skull), nasal revision, lip revision at 23 yrs. old (Children's Mercy Hospital)
Oct. 21, 1988 - Dilation of nasal airway with skin graft and placement of stent - left eyelid lift at 24 yrs. old (Menorah Medical Ctr.)
Oct. 20, 1989 - Reconstruction of columella with flaps (skin) from nose and placement of tubes in both tearducts at 25 yrs. old (Menorah Medical Ctr.)
Jan. 28, 1992 - Reconstruction of columella with skin flap from fold of right cheek at 27 yrs. old (Menorah Medical Ctr.)
Feb. 18, 1992 - Release of flap at 27 yrs. old (Menorah Medical Ctr.)
May 20, 1996 - Opening of nostrils more at 31 yrs. old (Baptist Medical Ctr.)
Sept. 8, 1997 - Nasal endoscopy, hardware (wire) removal, and revision to pharyngeal flap at 33 yrs. old (St. Luke's Hospital)
April 7, 1998 - Nasal reconstruction - opening up nostrils at 33 yrs. old (St. Luke's Hospital)
Jan. 12, 1999 - Cartilage graft from left ear to columella and repositioning of columella (St. Luke's Hospital)
